I've just discovered how to turn on ClearType in Windows XP. Control Panel -> Display -> Appearance -> Effects, then set "Use the following method to smooth edges of screen fonts" to ClearType. It greatly improves font quality on high-resolution LCD screens.

I'm still playing around with font sizes and DPI settings to get the ideal setup for my middle-aged eyes.
